Factors that decrease access to or availability of primary care or outpatient medical services, diabetes self-management services, nutritional counseling services or hospital services increase a community's or a groups' risk of illness. These factors include no or limited existing services nearby, lack of insurance, and fragmented service delivery or care systems.
